The Weekly Five | 12–18 October

Five studies worth your time this week, spanning randomized breast radiotherapy, secondary malignancies, adjuvant treatment decisions, long-term toxicity and proton selection.

1. RT CHARM confirms noninferiority of 16-fraction postmastectomy RT

Hypofractionated postmastectomy RT was noninferior to conventional treatment for reconstruction complications (12.3% vs 10.2%), supporting a shorter three-week course.

2. Proton therapy is associated with fewer second cancers in retinoblastoma survivors

At 15 years, in-field second malignancy incidence was 0% after proton RT versus 11.1% after photons, although the comparison was nonrandomized.

3. US consensus defines adjuvant RT indications for high-risk cutaneous SCC

Experts supported postoperative RT for unresectable positive margins and selected high-risk features, while elective nodal irradiation remained unresolved.

4. Five-fraction breast RT with boost shows low fibrosis but more pain

Five-year fibrosis was 8.9% after one-week breast RT with simultaneous boost, while breast pain was more frequent with boost in nonrandomized comparisons.

5. Updated toxicity model supports proton selection in lung cancer

A Dutch model predicted acute esophageal toxicity with AUC 0.75 and now informs proton selection, although clinical benefit remains unconfirmed.
